HIV transmission remains a major public health concern and affects more than 2.3 million people in the WHO European Region, particularly in the Eastern part of the Region. This report is the latest in a series published jointly by the European Centre for Disease Prevention and Control (ECDC) and the WHO Regional Office for Europe that has been reporting data on HIV and AIDS in the WHO European Region and in the European Union and European Economic Area (EU/EEA) since 2007. It finds that while epidemic patterns and trends vary widely across European countries, nearly 107 000 people were diagnosed with HIV in the European Region in 2022, including around 17 000 in the EU/EEA.

The efforts to expand the use of PrEP face challenges worldwide. As such, the
UNAIDS target of 3 million people having access to PrEP by 2020 globally, was not4
met. In Europe, there were 68 089 oral PrEP initiations in 2020.5 According to the
European Centre for Disease Prevention and Control (ECDC), "Greater access to PrEP
and progress in PrEP implementation is needed to reach the SDG of ending the AIDS
epidemic by 2030. To achieve this progress, it would be helpful for countries to gain
a better understanding of the barriers within their country to implementing PrEP
programmes. ”

From August to October 2022, AAE invited its member organisations and partners
based in one of the EU Member States to submit their responses on various
aspects of availability, accessibility, affordability, and acceptability (AAAA) of PrEP
in their national context.

This current report is based on the information provided in the survey and
reflects the practical knowledge on the issue of community-based and –led
service organisations that work with key populations. It includes besides the
main findings of the assessment, country profiles, as well as recommendations
for the European Union and its Member States for urgent steps to be taken to
ensure universal, free and low-threshold access to PrEP.

The UNAIDS Global AIDS Update 2021 highlights evidence that people living with HIV are more vulnerable to COVID-19, but that widening inequalities are preventing them from accessing COVID-19 vaccines and HIV services. The report shows how COVID-19 lockdowns and other restrictions have badly disrupted HIV testing—in many countries this has led to steep drops in HIV diagnoses, referrals to care services and HIV treatment initiations. Key populations and children continue to be left behind in access to HIV services.

UNAIDS and the World Health Organization have published this updated guidance on ethical considerations in HIV prevention trials. The new guidance is the result of a year-long process that saw more than 80 experts and members of the public give inputs and is published 21 years after the first edition appeared.

The HIV prevention goal set in the United Nations 2016 Political Declaration on Ending AIDS is to reduce new HIV infections to less than 200 000 by 2030. To this end, UNAIDS and WHO strongly support all valid approaches to improving HIV prevention technology, provision and use, while also recognizing their role in promoting the rights of populations affected by HIV. This revised guidance can be used to support all stakeholders in designing and conducting ethically and scientifically sound HIV prevention trials that advance the response towards the 2030 HIV prevention goal.

In 2019 an estimated 7.2 million people, 19% of all those with HIV, were undiagnosed. Men in high HIV burden settings and men from key populations are consistently less likely to know their HIV status than women. WHO recommends a strategic mix of differentiated HIV testing services (HTS) approaches to reach men and link them to appropriate prevention and treatment services.

This policy brief highlights WHO-recommended HTS approaches that programmes can introduce or scale up now to optimize the uptake of HTS among men. The brief also summarizes a package of post-test services for men. HTS can link men to these services as appropriate for their test results.

These recommendations have been developed specifically to address the daily use of antiretrovirals in HIV-uninfected people to block the acquisition of HIV infection. This prevention approach is known as pre-exposure prophylaxis.

At this stage evidence is available from studies with two groups: men and transgender women who have sex with men; and serodiscordant heterosexual couples. In parallel, WHO also is preparing new recommendations on the use of antiretroviral drugs in people living with HIV to prevent transmission of infection.

With the end of 2020 approaching, the latest data from countries show both progress and challenges. As the complex reality of HIV epidemics is increasingly understood, more HIV responses are sharpening their ability to identify gaps and develop strategies to reach people who are being left behind. However, too few countries have taken sufficient action to reach the interim milestones, leaving the world off track to achieve the three zeros by 2030.

Measurable targets were set for 2030, and the United Nations (UN) General Assembly later articulated interim 2020 milestones in the 2016 Political Declaration on Ending AIDS:

To reduce new HIV infections to fewer than 500 000 by 2020.
To reduce AIDS-related deaths to fewer than 500 000 by 2020.
To eliminate HIV-related stigma and discrimination by 2020.

UNAIDS report on the global AIDS epidemic shows that 2020 targets will not be met because of deeply unequal success; COVID-19 risks blowing HIV progress way off course. Missed targets have resulted in 3.5 million more HIV infections and 820 000 more AIDS-related deaths since 2015 than if the world was on track to meet the 2020 targets. In addition, the response could be set back further, by 10 years or more, if the COVID-19 pandemic results in severe disruptions to HIV services.

The Conference “Empowering Women Living with HIV in Eastern Europe” took place on November 22-24, 2019 in Minsk, Belarus. The conference in Minsk gave the participants a feeling of strength and inspiration, support and sisterhood, love and self-confidence. The conference was organized and conducted by Deutsche Aidshilfe (German Association of HIV Service Organizations), as part of the German Federal Ministry of Foreign Affairs program “Development of Cooperation with Civil Society in the Eastern Partnership Countries and Russia”.
